Data Source: Texas State Board of Dental Examiners - September 20, 2004

Includes: All Dental Hygienists with a license status code of "active" or "expired," and a Texas "primary address."

Excludes: Dental hygienists with a license status code of "surrender, revoked, retired, deceased, or canceled" (4,167). In addition, 812 dental hygienists were not included in this report because they had out-of-state "primary addresses." The hygienists were sorted by the county in which their "primary address" was located. The county was determined based on the zip code of the "primary address," which may be either the resident or practice address.
