Includes: Primary Care Dentists with a status code as "expired or "active" and a self-reported practice type of general dentistry, pediatric dentistry, private practice, or dental public health; excludes dentists with a status code of "retired," "revoked," "suspended", "N/A", "canceled," "surrendered," or "deceased" and those who are non-primary care. The Primary Care Dentists were sorted by their primary mailing addresses. The county was determined based on the zip code of the primary address, which may be either the home or practice address of the licensee.
