Excludes deceased, eligible for permanent, eligible for temporary, holding for documentation, expired, revoked, and suspended (214). In addition, 128 records were not included in this report because they had out-of-state addresses. The Occupational Therapists Assistant were sorted by their county of practice. The county was determined based on the zip code. If the practice address was unavailable, the mailing address was used to determine the county.
