Excludes deceased, eligible for permanent, eligible for temporary, holding for documentation, expired, revoked, and suspended (182) records. In addition, 380 records were not included in this report because they had out-of-state addresses. The Physical Therapist Assistants were sorted by their county of practice. The county was determined based on the zip code. If the practice address was unavailable, the mailing address was used to determine the county.
