CSHCN Services Program: Contract Providers

FY 2013 (September 1, 2012 – August 31, 2013)

The Children with Special Health Care Needs (CSHCN) Services Program contracts with community-based organizations throughout the state. Community-based organizations provide clinical and support services to children with special health care needs and their families.

Three categories of contracts are funded: Case Management, Clinical Supports, and Family Supports/Community Resources.
