Medical Home Child Health Day Video

The National Center for Medical Home Implementation has produced a video for Child Health Day 2009. The video features children, families, and providers who illustrate the value and importance of medical home for all children and youth. Other resources from Child Health Day are also available on the National Center's website.  

Fee-for-Service Reimbursement for Medical Home Service Providers

Clinician-directed care coordination services, including non face-to-face care coordination, are benefits of the Medicaid Comprehensive Care Program (CCP) and the CSHCN Services Program. Physicians, advanced practice nurses, and physician assistants may be reimbursed for these services. Care coordination services are payable to the primary care or sub-specialist clinician who provides the medical home for the client. Prior authorization is required for Medicaid providers, and authorization is require for CSHCN Services Program providers.

Emergency Preparedness for Children with Special Health Care Needs Booklet

The Children with Special Health Care Needs (CSHCN) Services Program of the Texas Department of State Health Services has published a bilingual booklet titled, “Emergency & Disaster Planning for Children with Special Health Care Needs / Planificación de emergencias y desastres para niños con necesidades especiales de salud” (2.42MB, PDF format, Publication Number 4-2, March 2008). The booklet can help families plan and be better prepared for an emergency or disaster. It has an "Emergency Information Form for Children with Special Health Care Needs / Formulario de Información para Emergencias de Niños con Necesidades Especiales" (316 KB, PDF format, Publication Number EF07-12540, March 2008) that the child's family and doctor can fill out to help the child receive proper health care in an emergency or disaster.

Medical Home Brochure

The CSHCN Services Program has a brochure available to help families and providers understand the importance of a medical home. This brochure is available in English and Spanish, and gives families and providers helpful information to remember when creating a medical home.

Texas Health Steps Offers Free Courses for Continuing Education Credits

Texas Health Steps Online provider education has developed a FREE, online program to teach health care providers about Texas Health Steps (Medicaid for children) and other health care services. The program offers FREE continuing education credits, and its courses are accredited. Topics include Introduction to Medical Home and many others!

The Primary Care Medical Home for the Chronically Ill Child Presentation

The powerpoint presentation reviews the rationale for medical homes for children with special health care needs (CSHCN), the central role of the Primary Care Physician (PCP), and strategies for successful “real world” PCP implementation of the CSHCN medical home, including practice management tools and coding for appropriate reimbursement.
