How to Enroll in Our Program

Enrolling in the CSHCN Services Program is now a click of the mouse away! Go to the Enrolling in the CSHCN Services Program page of the TMHP website. You can enroll online or print out the paper application form to complete and return to TMHP. If you have questions, call your local area TMHP Provider Relations representative. To find your area representative, go to the Provider Enrollment Regional Support web page or call 1-800-568-2413.

If you are interested in being a provider of respite care, home modification, home inspections, or vehicle modifications, click on the links below. You'll get the CSHCN Services Program Family Support Services (FSS) Provider Enrollment Application and Agreement as well as the Family Support Services Provider Manual.
