ADDITIONAL DOCUMENTATION WILL BE REQUIRED TO COMPLETE THE SALVAGE APPLICATION IF:

  1. Considered a charitable organization as described in the Internal Revenue Code 501 (c) (3),  please submit all required documents proof of 501 (c) exemption.
  2. If you are salvaging prescription drugs you will need to go to our website to download documents at http://www.dshs.state.tx.us/fdlicense.  Once on our website look for the "Additional Documents Required" tab on the left hand side menu.
  3. If you are salvaging non-prescription drugs, you must submit a list of licenses held in other states.

    ONLINE APPLICATIONS ONLY
  4. If applying online, you will need to submit the "APPLICATION SUMMARY" (which will be sent to your e-mail address) so additional documentation may be placed with your online application.
