Hepatitis C Support Grants Available
Vertex Pharmaceuticals Incorporated is launching a new grants program called Hep-C Circle of Care. This special grant program is independent of all other Vertex charitable grants programs and was specifically developed to support innovative collaborative care models that strive to achieve patient centered care for people infected with chronic hepatitis C.
The goal of this grants program is to fund innovative programs or tools being developed in the nonprofit community that:
* Provide innovative training and education supporting care coordination.
* Design or identify care coordination techniques in under-served communities.
* Create or employ tools used by health care providers to assess patient readiness for treatment.
* Build successful strategies in linking patients to care.
* Identify ways to overcome barriers to care including financial, psycho-social, & cultural issues.

WilCo Wellness Alliance Announces Grants to Support Community Gardens at Local Churches/Places of Worship in Williamson County
The WilCo Wellness Alliance announces a grant opportunity to the public from their Faith-Based Community Gardens Initiative. The WilCo Wellness Alliance is a local coalition in Williamson County that focuses on policy and environmental initiatives related to nutrition, physical activity, tobacco use, and chronic disease.
The goal of the Faith-Based Community Gardens Initiative is improved nutrition through education and sustainable Community Gardens. Small grants up to $325 will be awarded to five churches in Williamson County to help jump-start a community garden. The vision for these gardens is to create a space that will promote and support a healthy lifestyle for people in the community as well as serve as a benefit to those with limited food access. As a condition of this award, community gardeners in Williamson County agree to contribute 10 percent of their yield to charitable organizations.
Grant applications are available now and are open to any church/place of worship within Williamson County. Applications will be accepted by mail, e-mail, or may be hand delivered. The deadline to submit applications is September 1, 2011. PURPOSE: The purpose of the Minority AIDS Initiative Retention and Re-Engagement in HIV Care Project is to enhance access to and quality of HIV clinical care and support services for people with HIV disease who are traditionally underserved. The initiative's goals will be accomplished through the provision of training and technical assistance, which are defined for the purposes of this announcement as the identification of need for and delivery of practical program and technical support to the Ryan White HIV/AIDS Program community.

$ AVAILABLE: Three projects will be selected by the Changemakers community to receive an unrestricted cash prize of $10,000. Entries received by the early deadline of August 17, 2011, will be eligible to win an early entry prize of $1,000.
ELIGIBILITY: To be eligible for an award, entries must have proven success on some level and the potential to scale; or indicate a path for growth beyond the conceptual stage and have already demonstrated impact and sustainability. The competition supports new ideas at every stage and encourages their entry; however, the judges are better able to evaluate programs that are beyond the conceptual stage and have demonstrated proof of impact.
PURPOSE: The Making More Health: Achieving Individual, Family, and Community Well-being competition, launched by Ashoka's Changemakers in partnership with Boehringer Ingelheim, seeks to recognize innovative efforts that are transforming the field of health and increasing the well-being of individuals, families, and communities.
The Making More Health competition is open to individuals, teams, and organizations (charitable, public, or private entities). Anyone in the world may submit a solution or nominate a project that is transforming the field of health. The competition welcomes solutions and collaborations along the entire health continuum, from prevention and lifestyle to detection and treatment. Eligible initiatives may target a wide variety of populations and include (but are not limited to) those that increase access to quality health services and treatment; empower individuals, families, and communities to address local health issues; and target vulnerable and underserved populations.
