Adults need vaccines throughout their life. Store your immunization records in ImmTrac, the Texas immunization registry. It’s a FREE, secure, confidential service that stores immunization records electronically in one centralized system. ImmTrac is available to Texans of all ages. Just follow these three easy steps to participate.
- Download and complete the ImmTrac Adult Consent Form (F11-13366) from ImmTrac.com.
- Gather and make copies of your official immunization records (historical and/or current).
- Deliver your completed ImmTrac Adult Consent Form and copies of your official immunization records to your local health department
