ImmTrac Overview
ImmTrac, the Texas Immunization registry, is a free service offered by the Department of State Health Services (DSHS). ImmTrac is a secure and confidential registry available to all Texans. ImmTrac safely consolidates and stores immunization information electronically in one centralized system. Texas law requires written consent for ImmTrac participation and limits access to the Registry to only those individuals who have been authorized by law. ImmTrac contains several million immunization records and continues to rapidly grow with increase participation.
The valuable information comes from a variety of sources including private health-care providers; public health clinics; Medicaid claims administrators, the Bureau of Vital Statistics (VSU), and Women, Infants and Children (WIC) clinics. Regardless of the number of sources, each client’s immunization information is consolidated into one electronic record. Authorized professionals such as doctors, nurses, and public health providers can access confidential data including client’s vaccination histories.
The registry is a major component of the DSHS initiative to increase vaccine coverage across Texas.
How does ImmTrac work?
When a health-care provider gives your child an immunization, and you consent to registering this information, the statewide immunization registry, known as ImmTrac, is notified. ImmTrac will keep an electronic immunization record on your child. Some information contained in the registry is the child's name, date-of-birth, address, the name of the parent or guardian, information on the shots given, and who gave them. Optional (but very helpful) information stored in ImmTrac is the child's Social Security number and mother's maiden name. This optional information helps prevent duplicate records from being created.

How does ImmTrac help protect my child? Or how does ImmTrac benefit me/my family?
Providers authorized to use ImmTrac can see what immunizations your child has already had, even if they were given in another city or county. So when you come in for shots, your child gets only those that he or she needs.
- This means that your child won't be under-vaccinated and more susceptible to diseases.
- Your child won't be over-vaccinated either, so he or she does not have to go through any more discomfort than is necessary.
- ImmTrac can also print out a shot record you can use to get your child into school or childcare.
- Using ImmTrac, immunization providers can remind you to bring your child in for shots that are due, or to notify you about shots that are overdue.
Who has access to ImmTrac information?
Your privacy is protected. Your child's immunization information is available only to persons authorized by law to see it. Only doctors, schools, childcare centers, and public health providers with ImmTrac-issued identification numbers and passwords may view the information.

How do I register my child for ImmTrac if an ImmTrac consent form was not completed at the hospital??
To register your child for ImmTrac participation, you must grant consent in writing. Consent is only required one time and is only valid until your child turns 18.*
To do this, you may:
- Download the Immunization Registry (ImmTrac Consent) Form (C-7)
Note: A separate form needs to be completed for each individual.
Complete the form, sign and date it. Deliver the completed consent form, along with copies of your immunization records to:
- Any health-care provider registered with ImmTrac
- A local health department or
- Mail to:

How do I remove my child/myself/my family from ImmTrac?
Consent may be withdrawn at anytime by contacting the Immunization Branch. To do this, you may:
- Call the Immunization Branch at (800) 252-9152 and request an ImmTrac consent withdrawal form.
- Download the Withdrawal of Consent and Confirmation Form (Stock #C-8, Rev. 02/11). Complete the form. Sign and date it. Send the completed form by mail to:

How can we get a copy of our shot record?
To get a copy of a shot record for yourself or someone in your family you must fill out the authorization to Release ImmTrac Form. To do this, you may Download the Authorization to release ImmTrac History Form (F11-11406).
Complete the form, sign and date it. Send the completed form, by mail to:
