Forms Tips - Common Mistakes When Submitting Forms
Incomplete information:
Not providing complete forms (blank fields on forms or forms without signatures).
Not providing the 8-digit Submitter ID when requesting web services for LabWare and Neometrics. The 8-digit Submitter ID is located on a Microbiology laboratory report or a Newborn Screening laboratory report.
Missing forms:
Not providing the Facility Security Agreement when submitting the Security Rights Form – Web Remote User Access form and Confidentiality and Non-Disclosure Agreement form.
Not providing the Confidentiality and Non-Disclosure Agreement form when submitting the Security Rights Form – Web Remote User Access form and Facility Security Agreement.
Not providing the Security Rights Form – Web Remote User Access form when submitting the Confidentiality and Non-Disclosure Agreement form and Facility Security Agreement.
