Clinical Chemistry submission and remote access requires a Medicaid 9-digit TPI (Texas Provider Identifier) number. If you require a new TPI number or need to verify your current number, please contact Texas Medicaid Healthcare Partnership (TMHP) at http://www.tmhp.com/Pages/default.aspx.
Newborn Screening and Public Health remote access requires a DSHS Laboratory-supplied 8-digit Submitter ID number. To request an 8-digit Submitter ID, to establish a new provider, or to update your facility’s information with the Laboratory, please complete and submit the form below.
