Live Online Search Verification

When you select the link above, you will be taken to a live database search page and verify a license. When you are finished searching, please use the back button of your browser to return to this website.

Please Note: On the search verification screen, you do not need to register, enter an ID, or password. Please select the link "License Search" on the top right side that will direct you to the search page.

Search for a Licensee Instructions

One suggestion is to select “Search by Name and License Type” instead of “Search by Name,” which reduces the number of records being searched and improves search speed.

Additionally, as an alternative to the live search feature, please refer to the Excel rosters on licensed individuals using the Adobe Acrobat link to verify a license status from the “Roster” websites.

Search by Name

  • Search for an individual by typing first and last name or search by typing the full name of the organization.
  • You must select only one category indicated by the circle.
  • Once you have selected your choice, type the licensee’s name or organization. An error will occur if you do not select one of the two categories (search by name or search by organization).
  • At the bottom of the page, you can select the range (from 5 to 30 licensees) to display on the webpage
  • A list of licensees will display, then select the link in order to view or print a verification.
  • The verification will include name, license rank, license status, expiration date, effective rank date, and addresses (for most license types, full addresses will not appear; addresses will only appear for facilities and establishments).

Search by Name for a Specified License Type

  • Search for an individual by selecting the license type, then enter the first and last name, or search by typing the full name of the organization.

Search by License Number

  • If you already know the license number, select the “Search by License Number” link, then select the license type and enter the license number.
  • Select the link in order to view or print a license verification.

Search by City for a Specified License Type

  • Search for the nearest program by entering the city of your choice.
  • Select the program which applies to you.
  • Then type the name of the city of your choice.
  • At the bottom of the page, you can select the range (from 5 to 30 licensees) to display on the webpage
  • This should bring most locations near you in the city of your choice.
  • A list of programs will display and then select the link associated with the program that fits your needs.

Search by County for a Specified License Type

  • Search for the nearest program by entering the county of your choice.
  • Select the program which applies to you.
  • Then type the name of the county of your choice.

  • This should bring all locations near you in the county of your choice.
  • A list of programs will display and then select the link associated with the program that fits your needs.

Find a Licensee - Rosters

The rosters on this page are provided in Excel, a format that can be downloaded, saved and manipulated on your computer. Making changes to a downloaded file will not result in any changes to the posted rosters, which are stored on our secure web server.

Please Note: The rosters are updated infrequently (generally, on a monthly basis), and each roster lists the date it was created.

If you are needing to take an Offender Education Course, please use the Live Online Search link above or select the appropriate roster below.

Please contact the program of your choice directly to register, pay registration fees, or determine their schedule.  DSHS does not handle registration or have program course schedules.

***NOTE*** There has been a change in the format of the rosters for Education Programs. Previously the rosters were broken down by the type of education offered and whether or not it was offered in Spanish.  This is no longer the case.  From this point forward, with the exception of Texas Youth Tobacco Awareness Program, all programs will be on a single roster.

The programs will be listed in the order of the city in which they are located.  Column “B” will tell you the type of education that is offered.

To find programs offered in Spanish, perform a descending sort on column “N”.  This will bring all programs offered in Spanish to the top of the list.

      Special Message for Military Personnel, Spouses, and Veterans

      The State of Texas recognizes the contributions of our active duty military service members, their spouses, and veterans. The Texas Department of State Health Services (DSHS) in collaboration with the Office of the Governor, Texas Workforce Commission, Texas Veterans Commission, and Texas Department of Licensing and Regulation, is spreading the word to military personnel, their spouses, and veterans about licenses they may be eligible for and that may assist them in their search for work.

      The Professional Licensing and Certification Unit provides regulatory oversight, including licensing and certification, for the 23 allied health occupations identified on this page. Military personnel or veterans may have met some licensing requirements as a result of their military service or other training , knowledge, and experience. Military spouses of current, active duty military service members may also have met some licensing requirements related to this status. 

      Military personnel , their spouses, and veterans who are interested in one of the PLCU occupations and would like additional information regarding requirements may contact us via the contact information below or by clicking on the desired occupation for program specific contact information and a link to the program rules.

      While a military service member is deployed minimum requirements for license renewal may be modified.

      We extend our heartfelt thanks to military service members, veterans, and their families for their dedicated service to the United States of America and to the State of Texas.

      Enforcement Actions

      Disciplinary actions are taken by regulatory authorities (boards, committees, programs) in response to a violation of the law or rules. may be taken depending on the severity of the offense, the history of previous violations, efforts made to correct the violation, the harm to the victim(s), the likelihood of a repeat violation, and other matters related to each particular case.  These actions are taken to protect and promote the health, safety, and welfare of the citizens of Texas.

      Final Disciplinary Actions - License Denial, Surrender, Revocation 
      Further information, including a copy of the final order, is available upon written request from our office. 

      Final Disciplinary Actions - All Other, including Probated Suspension, Reprimand, and Administrative Penalty 
      These actions will be posted on this website for a total of seven (7) years from the date all the terms of the Order have been met, in accordance with our records retention schedule. The regulated individual may now be currently licensed, in good standing, and practicing without any restrictions.

      Complaint Process

      An individual who wishes to file a complaint against a Licensed Professional Counselor may write to:

      or call 1-800-942-5540 to request the appropriate form or obtain more information. This number is for complaints only. Please direct routine calls and correspondence to the phone number and address on the "Contact Us" page.

      To file a complaint, you will need to complete the Complaint and the General Release forms.  Return both forms and any additional supporting documentation to the address shown on the complaint form.

      After the completed complaint packet is returned, you will be notified by letter acknowledging that your complaint has been received.  The complaint will be reviewed to verify that the statement indicates a potential violation of law or rules.  If your complaint indicates a potential violation, you may be contacted for additional information.  Should your complaint not indicate a potential violation of law or rules, we will also notify you.

      A complaint may be filed anonymously by not providing your name and address on the form; however, please be aware that anonymous complaints may be difficult to investigate and many are closed because insufficient information is provided.  When you provide your name and address, our investigative staff will be able to contact you for additional information which may be necessary to resolve your complaint.

      In some instances, a complaint may be investigated informally by staff.  In other cases, investigations are conducted by a Texas Department of State Health Services investigator.  The complaint may be investigated by telephone, mail or personal interviews.   Information and/or documentation will be collected that is relevant to the complaint.  After the investigation is completed, the report of investigation will be forwarded to the program for appropriate action.  You may contact the board or program office at any time regarding the status of your complaint.
