Provisional Licensed Perfusionist and Licensed Perfusionist must now apply for their initial licenses through the new Regulatory Services Online Licensing System. Select the link below to begin the online initial application process. When using the online service, initial license application fees may be paid only using a major credit card.

Once you submit an online application, your fees and information will be processed automatically and routed electronically to the Perfusionist Licensing Program.

Please submit the following documents to complete you initial application for review:

Official transcripts from an approved perfusion education program (AC-PE) and approved by (CAAHEP)
Jurisprudence Exam
ABCP Certification Letter if applying for a Licensed Perfusionist
Provisional Licensed Perfusionist must Statement of Supervision form
