Industrial X-Ray Equipment Registration

For a new Certificate of Registration for Industrial Radiation Machines, please submit the following along with the appropriate fee:

• RC Form 226-3 – Application for Registration of Industrial Radiation Machines.
• Radiation Safety Officer (RSO) qualifications.
  Submit documentation of experience, education, and/or training.
• RC Form 226-1 – Business Information Form.
• To determine the fee for your facility refer to Title 25 Texas Administrative Code (TAC) §289.204.
• For registration and radiation safety requirements for industrial x-ray equipment please refer to 25 TAC §289.226 and 25 TAC §289.228.

X-Ray Radiation Machine Service Company Registration

For a new Certificate of Registration for Radiation Machines Services, please submit the following along with the appropriate fee:

• RC Form 226-5– Application for Registration of Radiation Machine Services.
• Radiation Safety Officer (RSO) qualifications.
  Submit documentation of experience, education, and/or training.
• Qualifications for persons performing radiation machine assembly, installation, or repair, according to Title 25 Texas Administrative Code (TAC) 289.226(t)(3).
• RC Form 226-1 – Business Information Form.
• Include a fee of $290.00 according to 25 TAC 289.204.
• For registration and radiation safety requirements for x-ray radiation machine services, please refer to 25 TAC 289.226.
