For a new Certificate of Registration, the following forms must be completed to register your dental x-ray equipment. Submit the completed forms along with the appropriate fee.

• RC Form 226-2 - Application for Users of Radiation Machines in Healing Arts
• RC Form 226-1 - Business Information Form
• RC Form 42-3 - Radiation Safety Officer Information
• To determine the fee for your facility refer to Title 25 Texas Administrative Code §289.232, subsection g.

Dental Regulations

You should become familiar with Title 25 Texas Administrative Code §289.232, Radiation Control Regulations for Dental Radiation Machines. This is the only radiation rule that applies to dentists using x-ray machines. This regulation requires you to establish operating and safety procedures for your dental facility. Regulatory Guide 4.4, is a guide to help you develop procedures unique to your facility. You should also post this Notice to Employees.
