For a new Certificate of Laser Registration, please submit the following along with the appropriate fee:

• RC Form 301-2 – Registration Application for Lasers     PDF    (103KB)        WORD    (88KB)
  Only Class IIIb or IV lasers need to be registered.
  
  
• RC Form 42-L - Laser Safety Officer (LSO) Information      PDF    (32KB)    WORD    (53KB)
  
  
• RC Form 226-1 – Business Information Form      PDF     (330KB)      WORD    (55KB)
  
  
• RC 301-3     (pdf 11KB) – Written Notice for Laser Light Show in Texas
  Only if registering a Laser Light Show.
  
  
• Two year fee of $230.00 in accordance with Title 25 Texas Administrative Code §289.204   (pdf 123KB):
  $230.00 for human use, research, academic, and veterinary.
  $400.00 for industrial use and entertainment.
