Laser Hair Removal Update (Rev 06/28/2012):

We are now accepting applications for laser hair removal program. Please see below for instructions and applications.

Renewal of Certificates of Registration

For renewals of Laser Hair Removal Certificates of Registration for facilities and individuals, please submit:

Facilities – RC Form 302-4, RC Form 226-1, $1260.00 fee

Apprentice-in-Training – RC Form 302-5 , $50.00 fee

Technician – RC Form 302-6, $70.00 fee

Senior – RC Form 302-7, $100.00 fee

Professional – RC Form 302-8, $150.00 fee

For continuing education topics and requirements for individuals, refer to 25 Texas Administrative Code 289.302(r), page 302-24. To determine your specific continuing education responsibility, refer to your Certificate of Registration.

Do not submit continuing education documentation at this time. Keep for audit purposes. For auditing purposes, document topics covered, source of instruction (ex: website address, book title, institution of attendance, etc.), time covering material, and date.

Fee of $50.00 according to Title 25 Texas Administrative Code §289.302.
