This page will assist you in providing the correct information needed to amend or terminate your Certificate of Registration for medical and industrial x-ray equipment.

Mail all correspondence or forms to:

AMENDING YOUR MEDICAL CERTIFICATE OF REGISTRATION

When submitting a request to amend your MEDICAL registration, you may write a letter describing the necessary change or complete RC Form 226-2, indicating the change in item 3. Either way, always reference your registration number and include any additional information needed to process your request.

Name and/or Ownership Change - See Regulatory Guide 8.2:  "Guidelines for Issuing Certificates of Registration Due to a Change in Name and/or Ownership"

Radiation Safety Officer Change - Complete appropriate section of RC Form 42-3

Deleting an X-ray Machine - Complete transfer/disposal form RC 18-R.

AMENDING YOUR INDUSTRIAL CERTIFICATE OF REGISTRATION

When submitting a request to amend your INDUSTRIAL registration, you may write a letter describing the necessary change or complete BRC Form 226-3, indicating the change in item 9. Either way, always reference your registration number and include any additional information needed to process your request.

Name and/or Ownership Change

Radiation Safety Officer Change - Submit a letter requesting the change with documentation of training and experience with the type of x-ray equipment you are registered for.

Deleting an X-ray Machine - Complete the "Radiation Machine Transfer/Disposal" form RC-18-R.

REQUESTING TERMINATION OF YOUR CERTIFICATE OF REGISTRATION

To terminate a medical or industrial registration, complete the "Request For Termination of Registration" form RC-17-R.
