Fee Schedule for New Certificates of Registration

The table below outlines fees pertaining to the issuance of a single certificate of registration for industrial use.

Each initial application for a certificate of registration shall be accompanied by a nonrefundable fee equal to the appropriate biennial fee.  Fee payments by check or money order shall be made payable to Texas Department of State Health Services. 

Multiple Categories of Use

In the case of a single certificate of registration that authorizes more than one category of use, the total fee is the category listed in 25 TAC §289.204 with the highest corresponding fee.  If the certificate of registration has additional authorized use locations, the registrant shall pay an additional 30% of the highest fee category for each additional use location. 

Registration for Radiation Machine Services

Radiation machine services include but are not limited to: Assembly, Installation, or repair, demonstrations and sales, providers of equipment, and consultants of radiation machines. 

To obtain a new registration for radiation machine services, submit the appropriate forms with $290 and Radiation Safety Officer qualifications.

Out of state service companies are required to provide a 3-day notice of work in Texas. For an example notice see X-Ray Services Notice of Intent to Work in The State of Texas (PDF, 101KB). 

Biennial Fees for X-Ray Registration

Biennial fees must be paid every two years to continue operating X-ray devices. The fee covers all direct and indirect costs associated with licensing a company or individual.

To renew a Certificate of Registration, complete and submit the appropriate application and Business Information Form.

Reciprocal Recognition

To obtain a new registration for X-ray reciprocity, submit required forms with appropriate fee and additional information listed on the reciprocity requirements page. Reciprocity registrants are required to provide a 3-day notice of work in Texas. Reciprocity will not be granted to out-of-state service companies. 

Amending a Certificate of Registration

To amend a Certificate of Registration, complete and submit the appropriate Application within 30 days of any change.
