The Diabetes Program Specialist provides assistance to community groups using the "Diabetes Today" model for community organization to address public health needs.
The goals of the program are to identify geographic areas with a high prevalence and burden of diabetes and to target diabetes services to these high risk groups. Health promotion and intervention strategies are implemented for behavioral and systems changes in communities.
The school based health promotion program CATCH (Coordinated Approach to Child Health) is being implemented in elementary schools to promote increased physical activity and improved nutrition to prevent obesity and chronic diseases.
Coordination and technical assistance is also provided to communities to implement walking programs. Staff are available for presentations, technical assistance and training in general diabetes education as well as chronic disease self management. The Diabetic Eye Disease Program offers free eye examinations to eligible diabetic clients.
