Sanitarian Registration Program Apply for a New Registration - Apply By Mail

When submitting an application for registration as a Sanitarian, please attach the following:

1. Job description of past and present work experiences relating to sanitarian activities, including the employment dates of each work experience.
2. CERTIFIED college transcript(s) from all colleges/universities (original).
3. Small photograph.
4. Registration fee.
5. Application must be notarized.
6. If currently certified by NEHA as REHS/RS, attach proof of current certification
