Delayed Certificate of Birth Registration For Children 4 to 15 Years of Age.

For a child between the ages of 4 to 15 years :

  1. The Delayed Certificate of Birth (VS-122) signed by the parent; legal guardian, or legal representative; and

  2. Two (2) or more documents attesting to the date and place of birth, one of which must verify parents' names. Only one document may be a notarized affidavit of personal knowledge. At least one document should have been created within 10 years of birth.
