• Include Detailed Information in Subject Line (Exp. "Having Problems Electronically Verifying a Death Record in TER Death")
• Your Name
• Your Business or Facility Name
• The EDR number If we are dealing with a Death Record
• The Name on the Record
• Date of the Event
• Contact Phone Number
• Detailed information on the issue you are experiencing, including the events that led up to the issue
