
Texas Cardiovascular Disease and Stroke Partnership
The Texas Cardiovascular Disease and Stroke Partnership is the network of partners from across the state representing state and local public and private health care, university, non-profit, business and community sectors. The Partnership will work in coordination with the Texas Council on Cardiovascular Disease and Stroke to plan, implement and evaluate heart disease and stroke prevention programs and policies within the state.
Purpose and Process
The partnership was created to assess the current status of the public health infrastructure in supporting heart disease and stroke prevention in Texas, determine gaps and needs within the infrastructure, and develop a coordinated, collaborative and comprehensive statewide plan of action.
Goal Committees
The partnership has four standing committees:
Goal I: Goal I is committed to working to implement Goal I of the Texas Plan to Reduce Cardiovascular Disease and Stroke: "Texas will experience improved cardiovascular health and quality of life through the prevention of risk factors." The Committee has also developed a list of resources representing a compilation of successful programs across the State. Click here to access the Goal I Resource List.
Goal II: The Goal II Committee is committed to working to implement Goal II of the Texas Plan to Reduce Cardiovascular Disease and Stroke: "Texans will experience improved cardiovascular health and quality of life through the detection and treatment of risk factors." As part of this goal, the committee has developed Medicine Assistance Programs, a list of resources that can be used to assist providers and patients in locating free and low cost medications for patients. The list has been compiled through a web search of government and commercial listings and has been verified whenever possible by the Better Business Bureau.
Goal III: The Goal III Committee is committed to working to implement Goal III of the Texas Plan to Reduce Cardiovascular Disease and Stroke.: "Texans will experience improved cardiovascular health and quality of life through the early detection and treatment of heart attack and stroke." The Committee has developed a Stroke Public Education Campaign Tool Kit that can be used to assist healthcare providers and other community leaders to educate the public through evidenced-based public education programs tailored to appropriate audiences. The tool kit has a listing of stroke campaigns, templates for community action plans, recruitment letters, funding letters, news releases and evaluation forms.
Goal IV: The Goal IV Committee is committed to working to implement Goal IV of the Texas Plan to Reduce Cardiovascular Disease and Stroke: "Texans will experience improved cardiovascular health and quality of life through the prevention of recurrent events." The Goal IV Committee has developed a schematic of the systems of care for heart attack and stroke so that all care providers could be identified for potential inclusion in committee work:
