The Texas Cardiovascular Quality and Patient Safety Initiative unveils its physician tool kit with the release of a practice management and a patient education tool.
The CVD Patient Tracking Form is aimed at closing the communication and treatment gaps prevalent in today's health care system. The Prescription Pad is a patient education piece aimed at increasing patients' awareness and compliance with cardiovascular secondary prevention treatments and guidelines.
