Licensing Requirements for Certified Food Manager Programs

Licensing Requirements

To become licensed as a certified food manager training program, please visit the Food Manager Forms and Applications page and complete the appropriate application for your program.

Return both the completed application and non-refundable fee made payable to:

Please allow 4-6 weeks processing time from date of submitting application.

Licensing Fees

Classroom Programs

Licensing Fee - $600.00
Late Fee - $100.00

Internet Examination

Licensing Fee - $2,000.00

Testing Sites

One Site: $400.00
2–10 Sites: $1000.00
Over 10 Sites: $2,000.00
Late Fee - $100.00

Renewal Licenses

Renewals are valid for two years from the anniversary date. Failure to submit the renewal fee before the expiration date will result in a delinquency fee for each location and must be remitted before the license or permit will be issued.

Initial Application

Instructor Application(s) - Classroom
Program Curriculum (14 hr) - Classroom

Renewal Application

Instructor Application(s) new & renewals - Classroom

Military Personnel

Confirmation of service - Classroom
Documentation of comparable training applicable to Instructor - Classroom
