Revised THMP Application for Assistance now available
A newly-revised version of the THMP Application for Assistance is now available for downloading in the documents section here, along with the accompanying page-by-page instructions. This revised version is meant to address as many areas as possible that tend to remain incomplete or unanswered when a patient's application is initially submitted to the program, placing that application in the "pending" file and delaying determination of eligibility status while we await receipt of the missing information. Remember, the more complete and thorough an application we initially receive, the faster we can process the application; when in doubt as to how to answer a specific question, feel free to call us weekdays at (800) 255-1090 from 8:00 am to 5:00 pm CST and we'll be happy to assist.
THMP Client Recertification Process Now Underway
The THMP is currently undertaking a recertification of all clients receiving program services, as requested by community advocates and originally mandated by our federal funding source. Recertification ensures that program files are current and accurate, and that all program recipients continue to meet eligibility criteria.
The recertification process requires clients to update and submit current household, income and insurance status information on the special application titled "Renewal Application for Medication Assistance." Please note that proof of income and insurance coverage are requirements for recertification; applications will be considered incomplete if such information is not included when applicable.
If a client is due to be recertified, the THMP will mail the renewal application, instructions and a letter informing the client about the recertification process. Please remember it is the client's responsibility to keep the THMP informed of any of the following changes:
* Address and Phone Number
* Household Information/Composition: such as marriage, divorce, changes in dependents
* Household Income Changes: such as returning to work, receiving Social Security or other benefits
* Insurance Benefit changes: Obtaining insurance (including Medicare Part D) or any changes in prescription coverage
