People wanting to participate in the Freestone County Community Health Needs Assessment have until Oct. 19 to complete the survey.

The Texas Department of State Health Services (DSHS) and several community health partners are assessing community health needs in Freestone County, gathering information on public health, mental health, substance abuse and medical care that can be used to improve community education, prevention, care and treatment.

Participants can complete the anonymous survey in English or Spanish online at www.dshs.state.tx.us/freestone.

People who need a paper copy of the survey sent to them may contact JoLeen Eiklenborg by calling (254) 778-6744 or sending an e-mail to joleen.eiklenborg@dshs.state.tx.us.

As of Oct. 9, 304 people who live and work in Freestone County have completed a survey.
