Obtaining a Certificate of Registration
For a certificate of registration for dental radiation machines, submit the following forms with the appropriate fee:

The table below outlines fees pertaining to the issuance of a single certificate of registration for use in dental radiography.
Fees are established in 25 Texas Administrative Code §289.232, and in 25 TAC, §289.204.
An updated certificate of registration will not be issued based on payment of biennial fees.

|Out of State Reciprocal Recognition
Each initial application for a certificate of registration shall be accompanied by a non-refundable fee.
Fee payments by check or money order shall be made payable to Texas Department of State Health Services.
Additional Use Location
If the certificate of registration has additional authorized use sites, the registrant shall pay an additional 30% for each additional use location.
Biennial Fees for X-Ray Registration
Biennial fees must be paid every two years in order to continue operating X-ray devices. The fee covers all direct and indirect costs associated with licensing a company or individual.

Amending a Certificate of Registration
When submitting a request to amend a dental registration, complete RC 226-2, Registration Application for Users of Radiation Machines in Healing Arts, Veterinary Medicine and Academic Facilities (PDF, 27KB), indicating the change in item 3, or, submit a letter signed by the RSO. Describe the change, reference the registration number, and include any additional information necessary to process the request.

Radiation Safety Officer Change - Complete appropriate section of RC 42-3, Radiation Safety Officer (RSO) Information for Registration (PDF, 301KB).
Deleting or Transferring a Machine
To delete or transfer an X-ray machine, complete and submit RC-18R, Radiation Machine Transfer/Disposal Form (PDF, 16KB).
Deletions with a replacement in the same category do not need to be reported. Keep all installation and transfer/disposal records on-site for inspection purposes.

Title 25 of the Texas Administrative Code (TAC) §289.232 requires each registrant to have and implement written operating and safety procedures.
