To obtain a new registration for industrial radiation machines, submit required forms with appropriate fee and Radiation Safety Officer qualifications.

Radiation Safety Officer (RSO) Qualifications
New Registration for Radiation Services
Radiation services include but are not limited to: Assemble/Install/Repair, Demonstrate and/or Sell, Consultant (LMP), Provide Equipment (rentals), and 40 hour Agency Accepted Training Course.
To obtain a new registration for X-ray radiation machine services, submit the appropriate forms with $290 and Radiation Safety Officer qualifications.
Required Forms
Complete and submit RC 226-5 and 226-1.
Include RSO resume, transcript, bio, or documentation of experience.

Out of state service companies are required to provide a 3 day notice of work in Texas. For an example notice see X-Ray Services Notice of Intent to Work in The State of Texas (PDF, 101KB).
Biennial Fees for X-Ray Registration
Biennial fees must be paid every two years in order to continue operating X-ray devices. The fee covers all direct and indirect costs associated with licensing a company or individual.

Registration Technical Renewal
To obtain an 8 year technical renewal, complete and submit the appropriate form below (RC 226-3 or RC 226-5) along with RC 226-1.
The agency will mail a renewal application within 45 days prior to the expiration date.
Reciprocal Recognition
To obtain a new registration for X-ray reciprocity, submit required forms with appropriate fee and additional information listed on the reciprocity requirements page. Reciprocity registrants are required to provide a 3-day notice of work in Texas. Reciprocity will not be granted to out-of-state service companies.
Registration Amendments
For a registration amendment, complete and submit the appropriate form below within 30 days:
Change of Company Name and/or Ownership
For company name change or change of ownership, complete and submit the appropriate form below (RC 226-3 or RC 226-5) along with RC 226-1:
NOTE: Some circumstances involving a change of name or ownership may require a new certificate of registration. See Regulatory Guide 8.2: Guidelines for Issuing Certificates of Registration Due to a Change in Name and/or Ownership (PDF, 27KB) to determine if a new certificate of registration is required.
Radiation Safety Officer (RSO) Changes
RSO Change for Industrial Radiation Machines
Complete and submit RC form 226-3 and completed RC 42-4 or RC 42-5 form.
RSO Change for X-Ray Radiation Services
Submit completed form RC 226-5 and add new RSO resume, transcript, bio, or documentation of experience.
Other Registration Amendments

Terminating a Registration
