Initial EMS Personnel Fees

Initial EMS Personnel certification/licensure application fees increased November 2004 due to the subscription fee charge mandated by the TexasOnline authority under SB1152.  The subscription fee increased all initial EMS certification/licensure application fees by $4 to $6, whether you submit an online or paper application.  View the new fee schedule below.

All EMS application fees are non-refundable and non-transferable.  Make check or money order payable to Texas Department of State Health Services.

Renewal EMS Provider and First Responder Organization Fees

Renewal EMS Provider and First Responder Organization license application fees increase on November 1, 2004. This is due to the subscription fee charge mandated by the TexasOnline authority under SB1152. The subscription fee will increase all initial EMS certification/licensure application fees by $10, whether you apply online or on paper. Check our web site for the latest information about fees before mailing your application to us. Updated application forms, with new fee amounts, will be available from filed offices on November 1, 2004, and on our web site soon. Applications received with incorrect fees will be considered deficient, which will cause a delay in your license. View the new fee schedule for providers and first responders.

Volunteer Fee Exempt Status

Fee exemption is allowed ONLY if you volunteer exclusively for a licensed provider or registered first responder organization (receive no pay from ANY service for providing EMS care). This does not include reimbursement for actual expenses for medical supplies, gasoline, clothing, meals and insurance incurred while volunteering. The EMS administrator is required to complete the Volunteer Sign-off section on your certification/licensure application.

If you submit an application fee, it will be considered a declaration of non-exempt fee status.

Volunteers are not exempt from fees when applying for inactive.
