We look forward to welcoming Samhita Ilango, the National Alliance to Advance Adolescent Health/Got Transition® Health Research/Policy Associate, as our featured presenter. Samhita will present "Integrating Health Care Transition Supports in Special Education." She will provide background and evidence for health care transition supports and share the latest tools and resources that you can use to build health care transition skills for youth and young adults receiving special education services.

Please join us to find out ways to partner with school teams to incorporate “health”, an area of transition that youth, young adults, and families don’t see coming in advance, into transition planning beginning at age 14. Bring your questions and feel free to share this announcement with others who may be interested!


Transition to Adulthood Learning Collaborative

The Transition to Adulthood Learning Collaborative (TALC) meets quarterly via webinar. A guest speaker presents on a transition topic. We also share information on upcoming events and new transition resources. All are welcome to join us, including:

  • Youth and young adults;
  • Parents and caregivers;
  • Case managers;
  • Health care plan representatives;
  • School transition specialists and teachers;
  • Hospital and university systems, and
  • Local community organizations.   

Membership is open to the public. Please email CSHCNSDG@dshs.texas.gov to join or learn more about TALC.
